L’hidradénome papillifère

We here report the case of a 25-year old primiparous woman presenting with painless bluish cystic lesion within the interlabial sulcus measuring 1.5 cm (A). Excision was performed under local anesthesia. Histological examination showed papillary projections coated by apocrine cells (B) suggesting the diagnosis of hidradenoma papilliferum (HP). It is a rare benign adnexal tumor, but it is the most common vulvo-glandular tumor (60%) observed in women between 20 and 90 years, never before puberty. It appears more often as a single asymptomatic lesion of bluish or red hot normal mucosal colour, located in the vulvar region within the interlabial sulcus or its adjacent zone. Histologically it appears as encapsulated cystic tumor, located in the deep dermis, without connection to the epidermis. This tumor is filled by conjunctive villosities, its lumen is lined by two layers of cells, one layer of secretory cells and one layer of small cuboidal cells with intensely basophilic nuclei (myoepithelial cells). It is probably an adenomatous proliferation of anogenital mammary-like glands. The curative surgical technique consists of simple exeresis.
